Know Your Dental Plan Options

Dental insurance policies help many people effectively budget for the cost of maintaining a great smile. Dental Insurance covers issues related to the teeth and gums, as well as preventative care such as annual cleanings. Certain cosmetic procedures, such as crowns or whitening may not be covered.
Dental and Vision plans are specific to your zip code and county. If the quote you request isn’t filtered to your area, you may not receive an accurate quote. That also means you may not be able to get the same plan as a friend or family member in a different area.

Know what you’re looking for in a plan

The suitability of a Dental or Vision plan is entirely based on you—your needs, your financial situation, and your provider preferences. Before requesting quotes, it’s useful to spend some time deciding what you’re looking for in a plan. Consider the following:

  • If you already have a Dentist that you like, and they are in the insurance company’s network, you’ll be able to opt for one of the lower cost plans (Typically an HMO).
  • If you don’t have a Dentist in mind, you can choose from one of the Dentists who are in the network and opt for one of the lower cost plans (Typically an HMO).
  • If your existing Dentist is not in the network, or you would like the freedom to go in or out of the network, you can still get Dental insurance, but you will pay more to see the out of network dentist and will need to opt for a PPO dental plan.
  • Most Dental policies also have waiting periods before any standard or major work can be done, typically ranging from 6 – 12 months.

By thinking about these factors before obtaining Dental quotes, you’re setting yourself up to more effectively evaluate the quotes you receive.

Understand the costs

Premium: the monthly cost of a dental or vision plan. Premiums vary depending on the insurance company, your location and the type of plan you choose (HMO or PPO).

Deductible: the portion of your dental costs you’ll pay before insurance pays (usually around $50 per person).

Co-pay/Coinsurance: the portion of each medical claim that you pay. Typically a set amount, such as $20 for this service, or a percentage, like 20% of each bill.*Most dental plans follow the 100/80/50 payment structure. They pay 100% for preventative care, 80% for basic procedures and 50% for major procedures.

Yearly Coverage Maximums: The majority of PPO dental policies cap the amount of annual coverage they will pay out on your behalf. Once the yearly maximum is reached you are responsible for 100% of any remaining dental procedures.

Privacy Notice Statement

The Federally facilitated Marketplace requires insurance brokers/agents to provide a Privacy Notice Statement to all Marketplace clients via a public-facing website or electronic or paper form before collecting Personally Identifiable Information (PII). This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Insuring Sources, Inc. is authorized to collect Personally Identifiable Information (PII) from you. Any PII we collect is used to assist with your health insurance eligibility, and may be shared or disclosed in the following ways:

Our Uses and Disclosures

We may use and share your information as we:


• Assist with your enrollment for health insurance coverage through the health insurance Marketplace and determine subsidy eligibility. The information you provide may be submitted to CMS (a Federal agency) and will be maintained in a Federal system of records.


• Help manage verifications and information requested by you, your insurance carrier, and the Marketplace, such as:

• Income, tax household size, citizenship or eligible immigration status,
permanent move, divorce, marriage, the birth of a child, adoption, social
security number, tax filing, and premium tax credit reconciliation, Federally recognized tribe status.


• Claim, billing inquiries, and premium payment information

• S.A. enrollment

• Other information we receive from you as requested on insurance enrollment
forms and applications.

• We can use and share your information to run our organization and contact you when necessary. We use health and demographic information about you to develop better services for you.
If you choose to give us PII, we may share this information with certain entities under the following circumstances:

• Comply with the law. We will share information about you if a state or federal laws
require it, including with the Department of Health and Human Services if it wants to
see that we’re complying with federal privacy law.


• Address workers’ compensation, law enforcement, and other government requests


• Respond to court or administrative orders, lawsuits, legal actions, subpoenas, and health oversight agencies for activities authorized by law.


• We may be required to collect specific personal information about you as required by the law and insurance carrier. If you choose not to provide us with the PII requested, or not to respond to specific questions, your enrollment may be denied or delayed, and we will inform you of such consequences

Our Uses and Disclosures

• We are required by law to maintain the privacy and security of your protected health information. 

• We will let you know promptly if a breach occurs that may have compromised the privacy or security of your data. 

• We must follow the duties and privacy practices described in this notice and make the information available to you. 

• We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind

Changes to the Terms of this Notice

We can change the terms of this notice, and the amendments will apply to all the information we have about you. The new notice will be available upon request and on our website.

Producer Compensation Disclosure

Compensation includes: commission, fees, awards, overrides, bonuses, contingent commissions, loans, stock options, gifts, prizes, or any other form of valuable consideration.

Regular commissions: Most revenues come from regular commissions paid on policies placed through Insuring Sources, Inc. varying by product and insurer. Commissions change annually and will range from a percentage to flat dollar amount and can also be paid based on the number of family members enrolled.

Overrides: few of the insurers that Insuring Sources, Inc. also grants Insuring Sources, Inc. the opportunity to earn an override. These overrides are based on Insuring Sources, Inc.’s collective volume of business. These overrides are modest and are used for supportive
services.

PRIVACY STATEMENT

What is this announcement about?

We value our customers and your trust in us and protecting your privacy is of extreme importance to us. This notice describes the data we collect and how we use, share and protect that data.

What DATA is collected and from whom?

We collect Personal Identifiable Information (PII) from the following sources: Information provided on your applications and ancillary forms; Information about your transactions with us, and our affiliates; Information we receive from third parties, including contracted and non-contracted agents, agencies, and insurance carriers. This information can be collected in person, via phone communication, by mail, fax, email, and other necessary electronic means.

What is DISCLOSED and to whom?

Your PII will be used and/or disclosed only o the extent necessary for the person’s or entities intended purposes and has not been used, altered or destroyed in an unauthorized manner.

Persons or companies that perform services on our behalf; Other insurance institutions with which we have joint marketing agreements as permitted by law; Other insurance support organizations for use in connection with an insurance transaction or to prevent fraud; An insurance regulatory authority; Law enforcement or other governmental authority to prevent or prosecute fraud or other unlawful activities;

The Health Insurance Marketplace and other governmental authorities, such as the IRS, CMS, ACHA, as required for the purposes of obtaining insurance benefits and tax premium subsidies; If applicable, an insurance carrier or the federal government for reporting claims or as a response to an audit.

We do not disclose any nonpublic information about you to either our affiliates or non-affiliates for any purpose other than to provide you with insurance products and services.

Your Right to Access, Verify and correct your PII. You have the right to request reasonable access to your nonpublic personal information. You may not access your personal information that relates to and is collected in connection with or in reasonable anticipation of a claim or a criminal or civil proceeding. If you believe the information we collected about you is inaccurate, you have the right to request in writing that we amend, correct or delete it. We are unable to request or implement changes to erroneous information that was submitted to insurance entities or the federal government on your behalf.

Our Security Procedures

We restrict access to nonpublic personal information and only allow disclosures to persons and companies as permitted or required by law to assist in providing products or services to you. We maintain physical, electronic, and procedural safeguards to protect your nonpublic personal information. Should your relationship with us end, we will maintain and only disclose your nonpublic personal information in accordance with this Privacy Policy as required by law.
